The following chart details the revised HomeCertain Underwriting Requirements according to face amount and issue age for face amounts of $500,000 or less. These requirements apply only to those applicants who submit a completed Mortgage Certification Form or valid mortgage lead card along with the HomeCertain application, and whose requested face amount does not exceed 125% of their mortgage amount. This chart also details the specific face amounts and issue ages where the Agent Collected Urine HIV Test (Heritage Lab Urine HIV Kit only) can be used in place of the basic paramedical requirements (which may include Home Office Specimen & Blood).

Agent Collected Urine HIV Test (Heritage Lab Urine HIV Kit only) is an optional replacement for standard requirements on  applicable HomeCertain policies in excess of $250,000. Agent Collected Urine HIV Test (Heritage Lab Urine HIV Kit only) involves the simple collection by the agent of a urine sample at the time of the application that is forwarded to Heritage Lab to be tested for various underwriting concerns including nicotine, cocaine and HIV. This process is painless and takes only takes a couple of minutes to complete.

Agent Collected Urine HIV Test kits (Heritage Lab Urine HIV Kit only) can be ordered through www.heritagelabs.com. Agent Collected Urine HIV Test Kits (Heritage Lab Urine HIV Kit only) include an instruction sheet, information and consent form, an HIV/AIDS brochure and a postage paid return envelope to send the attained specimen to Heritage Labs. The specimen and consent form should be mailed to Heritage Lab.

Agent Collected Urine HIV Test (Heritage Lab Urine HIV Kit only) kits include an instruction sheet, an information and consent form, an HIV/AIDS brochure,  and a postage paid return envelope to send the attained specimen to Heritage Labs. The Lab Copy (top copy) of the information and consent form should also be mailed along with the specimen to Heritage Labs. The Insurance Company Copy should be submitted to F&G Life along with the HomeCertain application.

Important Note: A state specific HIV consent forms must be submitted with these applications.

Effective immediately, OM Financial Life will accept a completed Mortgage Certification Form (Form FGLI 4623) as adequate mortgage documentation on all HomeCertain applications. Use of this form will simplify and standardize the HomeCertain sales process, and will also help to expedite policy issuance.

The Mortgage Certification Form is also available through the Sales Materials section of SalesLink and may be photocopied.
